Not sure when this bug first appeared, but it hasn't been fixed in the most recent patch. For some reason, sims will remain in the shower (even after the hygiene bar is full) and the action timer will still show 0% complete. Most times, if cancelling the action doesn't work, I have to reset the sim to get them out.
