Sims quite frequently complain that they need to do something with the toilet similar to when they have to pee but none of the behavior like holding it in. They hold their stomach, wave like they need attention and the thought bubble shows the toilet. Either sending them to the bathroom or using cheat needs make happy will make it go away. Frustrating because they're typically in the middle of engaging with another Sim.
This happens quite frequently. I've only seen with adults but my current families don't have a lot of kids over toddler stage.
Most recently I bought the life / death extension. I don't use and never used mods with my Sims 4. No other special function besides testing cheats.

I don't know how to trigger it so can't add an attachment. I'll try to capture it.

Same, thankyou for reporting this. I think it’s a bug from the last update, I have all the packs. It doesn’t really affect the sim but it’s very annoying.

I have most packs and this just start happening to all of my adult sims since the newest update. I started a family and so far the infants and the toddlers haven't seemed to act the same way. I have one that will be aging up to child soon so I will update if they start doing it too.

I'm on PS5 and I have it happening. Waves arms like hungry or rubs stomach and the toilet bubble comes up. His needs are full.
