I am wondering if this is related to the portrait outfit glitch?

For me this occurred with a sim who had just aged up from toddler to child, when we traveled to another lot. She was wearing her child outfit in the main view, but in portrait view she was still a toddler with her toddler outfit on. In the main view she had weirdly stretched limbs like in these pictures, and also was walking like a toddler. Then I had her change outfits a couple times and everything resolved itself.
I could see that. I did notice when it was the teenager, child, toddler, and infant, their portrait down below either showed their previous age or no portrait at all.
Though, there were a few times that my toddler looked like an infant walking and climbing on the couch a few times.
