It seems to happen all the time, even when not in a flirty mood they constantly do this, with everyone it seems, so not with someone in particular either.
Even a loyal married sim did it. Repeatedly. It was the main reason for their eventual divorce because she kept doing it.
No sims should ever automatically flirt when not in a flirty mood, and a loyal sim should never flirt with anyone else than their partner if in a relationship.
