This has happened intermittently in the past but consistently since today's update.  Vanilla save, only fixes I've got are standard cheats like "make happy".  Resetting the infant doesn't work, and neither does resetting the caregiver or assigning a different Sim to do the same task.  Exiting, clearing the cache, and opening up again makes it go away for a bit.
