I've tried to play a little bit and I also noticed that issue. I think it affect every situation and that it's an issue related to outfit generators (for example I observed it when a Sim switches into a towel to use the sauna - in this case the outfit generator is directly in the interaction - and for patients in the hospital - where the outfit generator is referenced in the situation job)
I love giving hats to my Sims, so that's really unfortunate 🥹
