Yes, it happens even when I move the child to another world — it still has the mask. Unfortunately, I don’t have a photo, but if it helps: there were four children, three that I created (all from different worlds) and they all had the masks, and one boy from Gibbi Point — I also play as his family — and he didn’t have it. Also, one of the children not only had the mask but was walking like a vampire… with that vampire walk style. Otherwise, they were using the playground object, and I could talk to them normally.

Asked around on this and CGrant56 pointed out that they are hats so what you describe sounds like a situational outfit issue.

crinrict hmmm I would agree with you but it seems to happen to sims that do not have hats on that outfit, for example I could invite one of my sims over for a chat and she would be wearing that yellow rain coat hat and I know its not right because that sim I invited over is from another one of my households I play and I know there is no hat with that outfit.

yeah, they have hats whether or not they should have them or not.
It's not a matter of a certain hat being switched for another. They just have random hats on their outfits for whatever reason.

crinrict arh yes then! even the maids are wearing hats now but it seems to be the silly bag on the head everyone is wearing now, ugh it's getting very tiresome all these issues😔

Thanks everyone. One more question — if those random hats and accessories appear on someone you’re not currently playing with but who belongs to one of your households, can you remove them from their head? As I mentioned, I can’t — they don’t show up in CAS, and when I use “Change Outfit” in live mode, I can see they’re equipped in every outfit. Luckily, I didn’t save the game and quickly reset it, so they don’t have them anymore. But honestly, I’m now afraid to travel anywhere that has playgrounds or interactive objects.

cicounek for me when I go back to that household they do not have them on, its only when I'm not playing with them and they visit or I see them out and about even the NPC have them on when I'm out.

That's been my experience. I've been trying to paint self-portraits of my Sim's various career outfits. But she returns from work wearing her astronaut helmet, and I can't remove it in CAS.
It doesn't seem to affect her interactions with other Sims, but it sure is annoying me.
