Playing on Mac, this seems to have started with the July 1st 2025 update. Suddenly sims such as hired caterers as well as with situational outfits like wedding outfits and hospital patients are wearing random hats with the regularly generated outfit. I do not have a screenshot of all of the instances but for example, the caterer at my sims wedding was wearing a beanie and the groom was wearing a graduation cap.
What I do have a screenshot of is a patient at the hospital during an active work day in the doctor career, she changed into the gown and also a clown hat (which is funny but I know is not intentional). She is also one of my sims so I know it's not part of any of her outfits so that is not a factor.
I do have mods installed (better build buy, ui cheats, and mccc), not sure why any of those would be causing this suddenly since it has never been an issue prior to this update but can of course test without them if need be.
