Having this issue today as well, have an alien sim. Tattoos appear separate in CAS but do not stay separate for disguised and alien form in-game. One set always overwrites the other, unsure of the order but it's happened both different ways where the alien form gets the disguise's whole set of tattoos and vice versa. 😔
