I have the same issue. The ears are zigzag shaped, one eye is missing or peak out a little through the cheek.
For me it seems to be only related to sims who get bit by townies werewolves. Maybe game generated NCP werewolves might be affected as well?
I had one that got a distorted werewolf form, but his human face got changed as well (but not "weird" looking, just another eye shape, nose preset etc). I'm not sure if that NCP was game generated as a werewolf or if one of my sims turned him. I deleted him because I got spooked he was corrupt. Hence he fuelled me into resetting the TS4 folder etc, but the issue persists with other sims.
Now my own werewolves are getting messed up. I had father winter get turned by the Volkov man and he had uneven eyes with one much larger and out of the head when he transformed (every time). He seem to get worse over time and I got nervous he would corrupt the game, so I cured him, but not before he had twins. The twins are normal.
My female sim also had a son with Howell from the werewolf pack and he is a teen now and look normal, but when I had the mother get turned by asking Rory for the cursed bite, she got a distorted werewolf form.
That is when I realised all my turned sims get this bug. I play on PC without mods and a few weeks old TS4 folder. The save I played in for above was new, I created it to play for this household.
