Audio issues on Xbox

I am having audio issues in every game I play on Xbox, either the sound cuts out completely or I can only hear certain sounds, often I cannot hear my own gunfire or the footsteps of other people. Som...

Has there been another patch, as I've played some games today and not had any audio issues. It's been great! Nice being able to hear the enemy before they shoot me in the face.
