@Kamp_03 I searched for this error code in this forum initially most recent post just to add my solution.
My nephew asked me about this exact specific PS4 error code on his 1st Gen PS4 with 500GB mechanical hard drive. It is all over the internet and Playstation support won't help if out of warranty. Internet forums just say turn off "Convert Incoming Voice to Text Chat" or factory reset/initialise.
1) Gave it a clean factory reset using below. This did not fix the problem.

2) I had a spare SSD and replaced the internal drive using Sony's instructions. The problem still occurred just as frequently.

3) I popped the original hard drive back in but then used a USB 3.0 to SATA enclosure with same SSD then utilising inbuilt function to copy Apex from original to SSD. So the game runs off the USB SSD but the firmware/saved game profile still resides on original internal hard drive.
It has been one week and no crashes so far compared to 3-4 a day.
So not necessary a hard drive problem, but it COULD be. It seems to be in this case the motherboard side internal connector for drive becomes faulty, so just have to get USB 3.0 to SATA enclosure and separate hard drive (or SSD as they're cheap now but it only seems to shave few secs off load times over USB so maybe not worth it)
Here is the weird thing. It only happened on Apex legends and not other games, although he mainly played this game I suppose 6+hrs a day.
