My speaker disappear and can't talk in-game

So every once in a while my little speaker icon next to my name will disappear, thus making voice chat impossible anymore. I don't know how to fix it, sometimes a reboot of the game works, sometimes it doesnt. Help please.

And also, the game keeps starting in windowed mode, even tho I always select full screen... this is annoying.
