Having the exact same issue. Can play 10 matches in a row with 27-45 ms ping and then suddenly 203 ms ping. I cannot count the amount of ranked points I have lost due to this 203 ms ping weirdness. I tried installing a tool to block all non EU servers which didn't help and now I'm also posting here. It started in season 12 for me. Never had the issue before season 12.
Since it's always 203 ms for me I have a feeling it's the same server I'm being connected to.

To see if I can find and block the high ping IP's

It looks like I have finally fixed the problem.
Using LiveTcpUdpWatch to look for the IP whenever I got into ~200 ms ping servers.
I found it was always an IP in the range of 185.225.209.XXX.
IP range 185.225.208.0 - 185.225.211.255 belongs to US-VPS-NET which is a VPS.net Cloud Service.
So no wonder the ping is high. Why Apex is connecting me to that IP once in a while I have no idea.
Solution :
I created an Inbound rule (in defender firewall) blocking 185.225.209.0 - 185.225.209.255, since the IP's I have seen was always in the 185.225.209.XXX. range.
I also created an outbound rule with the same IP range inbound rule on it's own didn't work. In the outbound rule I blocked any app/program/service not just r5apex.exe
After playing several hours in the last couple of days I haven't been connected to a single high ping game and I have had two code leaf events. Usually I never see code leaf.
So I will keep playing and if I get any high ping games I will update this post with the IP. But for now it seems to have solved the problem.

UPDATE
I haven't had a single high ping game since I blocked the above IP range in the firewall.
I have had 7 or 8 code leaf events, but NO high ping in pub or ranked games.
Sometimes I get 2 code leaf events in a row, but I don't loose any ranked points.
So for now it's working.

Thanks for this! I did it too and its fixed for me as well now. I get the Code LEAF error sometimes but thats much better than 200 ping games.- 4 years ago
Update April 30th 2022
Added 2 more IP's today
185.225.208.183 and 185.80.220.54.
When connected to 185.80.220.54 I'm lagging like crazy - ping is fine but I can barely move
So now I have the following blocked
185.225.209.0 - 185.225.209.255
185.225.208.183
185.80.220.54.
All of them are blocked in inbound for r5apex.exe and all of them are blocked in outbound for any app.
It doesn't work if the IP's are only blocked in inbound. They need to be blocked in outbound as well.
