How often does the bug occur?

Ever since I did a fresh install of windows, all of my drivers, software and even the game itself it hasn't occurred as often as it used to. Although it is still happening.

I also had to roll back my graphics drivers a little over a week ago due to the new drivers causing the game to instantly crash the second it hit the main menu screen where it shows your character.
How severe is your issue?

It's fairly severe when it does happen as it crashes the game, but sometimes it doesn't happen at all for a whole 8 hour session.

It may be random and happen every once in a while, but it's still a major issue for me to deal with.

Especially when I'm streaming and have to take the time to relaunch the game and queue back up with my friends.
What happens when the bug occurs?

The game freezes and pops up this error window, thus causing the game to close (crash).

There are also times where the game just freezes and doesn't say anything, nor does it say that it printed a crash file.
