Hi I am having a similar issue. The submit button is broken and I seemingly have no way to contact support. Is it possible to have the support team open a case and reach out to me?

Just to confirm, are you signed into the account with the account action? The appeal option won't be available unless you are. Please check your Connected Accounts page for the gamertag you play with and your Order History for past purchases to make sure you're on the right account. The EA ID should match what you see on the EA App and include the platform accounts you usually play with.
