Hi there. Thanks for including the DxDiag. Here are some suggestions that may help with this issue.

You're on a non-mainstream Windows build. Switch to a more stable version of Windows and make sure it's updated
Your Nvidia driver is very new. Try rolling back to a previous one
Disable overlays for the EA App, Discord, GeForce Experience/ShadowPlay, Steam, MSI Afterburner/RivaTuner, Overworld, Reshade and any capture/OSD tools
