Game crashing constantly

I am having a really persistent issue where the game crashes CONSTANTLY. It mostly happens when I'm starting a game, starting the second half or ending the game. Mostly in career mode but in happens in FUT too. Sometimes mid game. I have had the  game since around launch day and have put in about 20 hours. The game has crashed like 30 times. It's getting to the point that I don't want to play because it crashes so much and I've had to replay multiple career mode games because it crashed as I was hitting "advance" AFTER the game was over. I have uninstalled and reinstalled multiple times, I have cleared the cache and rebuilt the database multiple times. Idk if this is a memory leak issue but PLEASE FIX THIS!
