Completed Mass Effect 2 in the Mass Effect Legendary Edition on Insanity and was not awarded Insanity 2 for completing 2 mass effect games on insanity. I completed mass effect 1 on insanity and got the achievement. I started the game on insanity and played all the way through and completed on insanity. No achievement. I have started Mass Effect 3 on insanity and hopefully I'll get both achievements at the same time when I complete mass effect 3 on insanity.

Just wanted to confirm this is something we've seen and should have it addressed in a future patch.
there is actually a way around these 2 not unlocking.
For the paramour achievements/trophy they can be cheesed by loading up any save in ME1, then saving over that save, then exit to main menu. Then press start on ME2, and trigger the booty call scene. Achievement pops. If you repeat it again, you will get the paramour 3, though it is a glitch, that should only be patched after the achievement is confirmed to be unlockable as it is intended.
For insanity 2 and 3 there is a glitch that if you reload the autosave at the beginning of the saren fight, you can earn the insanity 2 and 3 achievements just by killing him again on insanity. Personally i am still going to play on insanity anyway for the consistency and i've played OG ME3 so many times on insanity im used to the difficulty.
However, it will not trigger the Long Service Medal achievement, and some people are mentioning on reddit that this achievement doesnt pop correctly for some people either.
Also, I don't know if it is related, but on the extras menu for all 3 games where it shows the achievements in game on the menus that show them, it doesnt show these achievements highlighted even though they have been earned.
Me also, and I’d rather not play thru me2 insanity again lol.
I restarted with the intention of completing insanity achievements without changing career (which I have an inkling is the problem).
Re-played ME1 on insanity (which popped 'Insanity II' on Xbox). Incidentally, re-playing the romance scene popped 'Paramour II'.
Yet the achievement log in my cabin and the Xbox achievements list are out of sync, but I'm less concerned by that - that should fix by patching.
So having this issue
So it did not pop for insanity 2 or 3 on xbox for me, however, i i decided to try something that was similar to workaround for paramour issue. After beating me 2 on insanity i went back to save file me1 and did final battle again. It popped insanity 2. Tried one more time just to see, didnt pop the 3 rd one. Went and beat me3 on insanity. No pop. Roaded game/ restart console. Did final boss me1 insanity. Third one popped. Its a pain doing it this way but final boss on me1 isnt soo bad so no big deal.
Jeepers, this better be ironed out before long, like many others I'm still rolling along on Insanity in ME2, if the Paramour II and Insanity II trophies don't pop...I'll turn into an absolute rabid crying Krogan on every single piece of Social media and forum there is lol.

I finished all 3 on insanity and i got all achievements for it, maybe at some point the dificulty was changed and thats why you did not get it, you need to start on insanity me 1 finish it without a change in dificulty, import that save in me 2, do the same thing and after that me3 and it should work on pc at least.
