Beat all 3 games on insanity a couple of weeks ago, romanced Liara in all 3 aswell but only received Insanity and Paramour 1. Still hasn't given me the other achievements.(Playing on Xbox) trying to 100% the game so it's quite the annoying problem😅
I just beat the entire Mass Effect Trilogy last night, 100%, all side quests, missions, DLC, everything beginning to end on Insanity difficulty and romanced Liara in all three games. Playing the Legendary Edition on PS5 and the trophies for romancing which are Paramour II and Paramour III didn’t unlock. The worst is that upon completing Mass Effect 3 on Insanity, I didn’t get the trophy for Insanity III. I never once changed any difficulty settings or anything in any of the games. Insanity on all three games was a challenge and quite stressful for certain parts. Please look into these bugs and patch this asap. This is my third time beating the Trilogy and I’d gladly romance other characters again, but I’m not in the mood to replay all of Mass Effect 3 on Insanity again. The Kai Leng fight and the Banshees in the final push on Earth are just broken with their one shot abilities.
I romanced Ashley in the first game and carried everything over into the second game. I romanced Miranda in ME2 and the Paramour 2 Achievement did not unlock. Afraid to play ME3 for fear of Paramour 3 Achievement nor working. Any help would be appreciated.

If so, it is a known bug that hopefully get fixed soon.
Yup, Xbox here. I'm reading that it's an Xbox issue. You'd think that after a month and a half of the game being out, this issue would have been fixed already.
It's not exclusive to Xbox. I play on PC through Steam and I can't get the Insanity II achievement to unlock. I've reloaded and replayed significant portions of the game (from prior to the Reaper IFF, to just before the actual final mission) with no luck. At this point I'm delaying my playthrough of ME3, since it seems likely that Insanity III won't pop, either.
Agreed, it’s not exclusive to Xbox. I can’t get Insanity III to unlock after completing ME3 twice on Insanity with two different endings and I’m playing the PS4 version on my PS5.

in that case the hotfix didn't change the patch numbering.
I saw the 300mb download and it fixed two of my achievements.
I know I have the latest version and that's its number - but as you say the number may be unchanged until the next proper patch.
