Beat all 3 games on insanity a couple of weeks ago, romanced Liara in all 3 aswell but only received Insanity and Paramour 1. Still hasn't given me the other achievements.(Playing on Xbox) trying to 100% the game so it's quite the annoying problem😅

I found a workaround at least for me on PS for insanity 2. Not sure if it would work for others but you can always give it a shot. I completed both ME and ME2 on insanity and didn’t get 2 after beating the game. Reloading the save prior to the fight with Saren also didn’t work for me. I had to take some additional steps from just beating him.There was an old workaround on last gen that I attempted and it worked. 1. load the fight with Saren (right before the second battle where he transforms)2. Complete the game3. reload the save and change the difficulty to insanity and make a new save4. Start a new game and import the character you just beat the game with 5 change to insanity while on the Normandy6. land on eden prime and make a new additional save7. reload the fight with Saren you changed the difficulty on and beat himthis got insanity 2 to pop for me, maybe it will help someone. Maybe it’s an issue where it’s not recognizing the imported difficulties across the games. From other posts I’ve read, it seems to be glitchiest when you import rather than beating all 3 games on a separate character.

Made an account just to post this. If anyone else is struggling with this achievement, follow shepardessia's advice above. I beat ME2 twice on insanity. No achievement. Went back and beat Saren three times in ME1 on insanity. No achievement. Followed the 7 steps above and boom: achievement.Thanks for the help!I'm about to start ME3 and am expecting to run into the same issue. Any idea if this workaround will work for Insanity III?

If only they could communicate about actual fixes as vigorously as they save us from big bad third party links.

Hey @Mr_Irishman, I'm seeing some indications that these may not be retroactively unlocked, but should pop if you're able to re-load any saves towards the end and re-complete the last mission. If you do have save files available that may be worth a try.

Thanks @abc1810443901. We are investigating those greyed out achievements on Xbox, is that what you're playing on as well?That issue may be contributing to some of these other situations, thanks for all the info everyone.

@EA_Mako tried this for both paramour and insanity 2, no luck. played through 2 twice now. both on insanity. no achievement. very frustrating, never had this issue with the original

Just completed ME3 on Insanity (playing on XboxOne). Confirmed my relationship (Jack) from ME2 and continued it into 3, and as title states, still no Insanity or Paramour 2, and also now 3 hasn't been awarded.
I did however get the Long Service Medal achievement so I guess thats positive.
Still loved the games still loved the challenge of Insanity, but not having the achievements pop is frustrating.
Please Bioware, fix this.

@rolltotheo jumping back to ME1 at least fixes insanity 2 (for now) no idea if it works for 3 though. bit doing it thisnway doesn't show in the actual game achievements, just on xbox

Those missing in-game achievements may indeed have an impact on these triggering properly

That's something that we're still looking into, so hopefully once that aspect is fixed it will properly reflect your efforts here as well.

No date to share on that yet I'm afraid, but thanks for the patience while we're working on it, everyone.

@EA_Mako well for now i'll just be clever and make plenty of saves prior to when they should unlock. then hopefully when theres a patch i dont have to go through it all again

Probably a smart idea! That's one of the workarounds that helped a lot of people prior to the latest issues popping up, so having multiple save points to go back to never hurts.

So I also had this issue with Insanity 3 not unlocking after beating ME3 a day after the patch. I tried a few different times to replay the end to no avail. Today I remembered I had saves on ME1 at the Saren fight and on ME2 near the end of the Collector base. I started with ME1 and killed Saren again, checked to see that the in-game achievement tracker now showed Insanity 1 & 2 as complete. I started up ME2 and beat it, and voila, Insanity 3 popped.
So to anyone at their wits end, maybe this could help you get this done and out of the way.

I finished the game last night, I romance Jacob , got the cut scene and not achievement pop, I heard it was a patch but did not fix .
anybody has any solution.. ☹️

I just finished the game, and Insanity III did not unlock for me either.
I played trough all the games in a row Insanity I and II unlocked properly.
Also, Paramour III is bugged too, but that can still be solved with the workaround glitch (insanity not).

Just completed the final mission on insanity for ME3 and still no achievement. Same for ME2. I was able to get paramore 2 and 3 on a different playthrough.
