Game freezes with "Active Controller Disconnected" message

As the title says, EA Sports PGA Tour has frozen on me, with a "Active Controller Disconnected" message (see attached image).  It says to "Press A on any controller to continue with that controller", but none of the buttons will respond.  I can use the controller to back out of the game to the main Xbox menu and all buttons work fine in the Xbox menus or on other games, but I'm completely frozen out of EA Sports PGA Tour.  I've rebooted both the controller and the console, but every time I open up the game I come back to this message and I'm completely locked out.  Thoughts?
