You can email the code into support@swtor.com and also email in the purchase email showing what was purchased
It is possible your swtor account is presently disabled for purchasing, in these cases you cannot buy a subscription on swtor.com, steam purchases will not add any gametime and codes will fail
Checking your account for issues, response will be in about 9 hrs when support are online for the day
As to the displayname it cant be changed at present
