adopted pet never showed up, but their icon is on my household panel

I adopted a dog from shelter, but it never showed up.. Just the social worker did.. And the pet icon on the panel but with no picture. I tried resetting sim cheat, I restarted game and while the dog showed up in the family photo it didn't show up in the game. Yet it showed up in my relationship panel as well. Very confused, ended up losing whole game. Anyway way to.prevent.this from happening again??
