This sounds alot like the rebellious Nannies issue from years ago, dont know if nannies are part of this particular issue the original poster had. But if there is a Nanny involved, it might be the nanny behind it! 😠
Why I stopped hiring Nannies. They were becoming more of a hindrance than they were a helping hand. 🤷🏻♀️

I originally didn’t have a Nanny but once they died the first time… I exited out w/o saving & went back in && hired a butler first… Then it happen again… so i repeated & keep the butler and hired a Nanny as well… Even though Butler is selected to care for minors… It still happened again so i exited the third time… Keep the butler, Nanny & my sim from work and they still got taken for neglect😓 idk what to do… I don’t wanna age them up!! & they not even coming back as playable ghost so i can give them ambrosia😢
Sounds like this is a totally different issue from the old Rebellious Nanny issue there used to be. It got ugly, Nanny would stand there for hours while my sims were at work and the moment one of the parent sims wanted to see to their own infant/toddler the nanny suddenly was there. I thought for sure the Nanny and the parent were about to fight, that was when the children where whisked away for neglect. 😭
But this particular issue was years ago. I have not encountered the current issue, at least not yet.
