@Sefdancer4life I'm actually unsure if this is a bug or not. The option has started appearing always for withered stomach weakness and pregnant Sims a while ago.
There's a bug report here: https://answers.ea.com/t5/Bug-Reports/UNSURE-V-Throw-Up-Option-Always-Available-With-Withered-Stomach/td-p/6899420/ but I don't know if it's really a bug or not.
They won't throw up on their own in their second trimester though so if you don't do the interaction, you should be fine.

@Sefdancer4life @crinrict I don't think this is a bug. I believe the throw up option has always been there with pregnant sims and with the vampires with withered stomachs. The only real purpose that I see for the option on the toilets to throw up is to get rid of the negative buffs that come with pregnancy and withered stomach. If the sim throws up, then the negative buffs go away for awhile.
