Holding Nothing Glitch

I've been noticing that my sims are often holding up their hands with nothing in them. It seems like a book or a glass should be in their hand, but there's nothing there. I ignored it at first, but it seems to be happening fairly often.
I'm trying to narrow it down to a mod glitch or an actual game-play one. Has anybody else experienced the same?
