need help i don't have hospital lot like trait

i was trying to set up the hospital but I don't see a medical clinic in the world description. Can anyone please help?

    Unfortunately, there’s only one hospital and you only have it if you have Get To Work AND it’s only accessible if you’re in labor or in the doctor career.
