Could you please edit your post and fill out the bug form?
As you said you weren't prompted for it, Editing should help with that.

    Thanks for filling the bug form and providing a workaround.

    I see you mentioned you have mods installed, could you please confirm if you tried to reproduce the issue without mods/cc? If not could you please try to reproduce the issue without mods/cc installed?
