I notice that aging is off in the game sometimes, such as my sim will marry their teen love who aged to YA before them, yet after married and they move in they're younger than my sim. Howver this is the first time I've had a played sim de-age. This sim married into my sim family and was fine until it was almost time to age up to elder. I noticed her gap between her husband and self was larger than when they were YAs. I couldn't age her up when I wanted via cake the other day. Today I notice that she is now no longer an adult, but a young adult with 22 days left to age up! As I continue to play she is suddenly moved in the icons as she is now a day younger than her YA son. She just keeps on de-aging....I have MC Command Center installed and some CC wallpapers.

@lisastar08 I took a look at your sim and the original from a new game as well. In a nutshell, the aging problem is caused by Alex (Moyer is the original surname) being a special sim that is used in the tutorial. She has the hidden trait: trait_SpecialNPC_isFTUERoommate which adds Disable Aging and Disable Death to the sim. That explains a lot about why she has aging issues as the game wants to keep her a YA with a set number of days in her age group. Probably so that she will be there if players rerun the tutorial. It's a guess though.I did some further testing and like @SheriGR neither the original from my test game nor your Alex get the Blow out the Candles option on a cake to age them up. But I also went into CAS and created a clone of her. Actually, I used add a new sim through Play with Genetics and created a twin for both the original from my game and your Alex. The twin is not exact as it had different clothes and hair and her size was a little different but those are easy to change. If you use the cas.fulleditmode cheat before you take the household in CAS, you can also deal with the size difference by using the sliders.MCCC does not have a way to edit the hidden traits, only the original 3 that the player can assign at sim creation. In fact, MCCC does not even see them. I use a mod called ShowSimInfo by Itasan2 which you can find at MTS to see hidden traits. That mod can't change anything. It just shows all kinds of nice to know info about a sim. It's a nice complement to MCCC.But in the end, your situation comes down to the Alex sim in your game. The only way I can think of to fix the problem is to clone her by creating a twin in CAS and then manually copying her clothes, hair, makeup etc. there. (You can also use MCCC to copy all the appearance things but I haven't tried doing that so I don't know how it works. I just mention it so you know the option exists) If you have the cas.fulleditmode active at the time you create the twin, you can also address her body size and traits. Then delete your Alex and rename the twin. Then you can use MCCC to set the skills to what your Alex had and if necessary, set any relationships that you couldn't in CAS. MCCC will also let you change days in an age range and other things to get the new sim as close to the old one as you can.So If you choose to go that way, I suggest you make notes if you want to duplicate the sim to get the new one as close as possible to the old one since there's a lot of things that will need to be changed.Anyway, I hope this helps you.

@lisastar08 Thank you for uploading Alex. I have been chatting with @EuphorialQueen & @CGrant56 and they both have some ideas on this that I think are very helpful (plus I'm not feeling well) so I am tagging them to see if they can add some light to this. I'm told that Alex Moyer is in the tutorial and is a Maxis sim that is set to never age and never die. EuphorialQueen has played her in the tutorial, and CGrant56 has played with MCCC. Edit: I did download your Alex and had a roommate bake a cake and add candles, and the other sim got the option to blow out candles, but Alex does not have that option.

@CGrant56 Thank you for all of the awesome and helpful information!!@lisastar08 After sleeping on this I won't know what I was thinking to not also mention that since we now have confirmation the this is a 'special' sim and is forcing itself back to the age it was designed to be... I suppose that another option could be to go CAS and edit the physical traits of your Alex so that she looks older, even though she is not. Add wrinkles, grey or semi-grey hair, whatever will make her look the age you want her.An example of doing this is Vlad, from the Vampires pack. If you look at the screenshot below he obviously looks like an elder, but if you look at the sim age area at the top left he is actually a young adult:

You would have to cheat-create the relationships, career and money your sim currently has, though.Or... you could try to save a copy of this sim with the entire household into the library, then place them again or in a new game save. The only relationships that will be preserved in this case will be the relationships with the sims saved with them into the library.Let me know if this helps. @lisastar08 Thank you for the information.What I mean is saving the sim into your library per the instructions in my other comment (there are links in the blue text to directions), then using the directions for making a new game-save in that comment (also see the link), then entering the new game save and downloading your sim into a home in that game save and seeing if you can age her up there.If it works you could try re-uploading her to your library from the game save (the aged up sim), then enter your current game and re-place the sim in the household and see if she then functions/ages properly for you.

Do you mean another played household? Just tried that and it didn't work. This is all being done with no mods/cc.
I also noticed in playing a bit tonight that the sim goes from 21 days now to 20, 19, then went back to 21 and hasn't moved again for days.

Can you please try placing them from the library into another game-save (with no Mods/CC) and see if you are then able to age them up? Thanks!

I did the sim reset and the issue persisted, so I split the sim from the household, put them in the library and moved them into another household, they still cannot age up via cake. They also still had their memories and relationships in tact.
The sim is also not pregnant.

Thank you, I followed the steps and the issue is still present so I didn't make it to re-adding my Mods. The sim still cannot age up via cake, I tested to make sure another sim could and they can.

Please try the following in this order without skipping any steps, even if you’ve done some of these steps already:
Exit your game then exit Origin. Drag the Mods folder from your 'The Sims 4' folder onto your desktop (for future reference) then delete the localthumbnailcache.package file in your 'The Sims 4' folder. (When you restart the game it will automatically create a new Mods folder.)
Next please run a game repair [ Open Origin, click 'Game Library' then right-click Sims 4 > Repair Game ]. Make sure to let it run until it has completed all repairs (it will do the base-game first, then run through each of your expansions/packs).
Do not re-add any of your Mods or CC at this time. Please then restart your game and see if the issue is present.
If this fixes the issue you can re-add your Mods/CC one at a time to see which were conflicting with the function/causing the glitch - but do not re-add the old Resource.cfg file from the old Mods folder, as it is possible for it to have become corrupted.
If you have a lot of Mods or CC you can do so by via the 50/50 method. Let me know if you need directions on this.
