My household are bald and in their underwear, but only on the home screen. When I load into the save, everything is normal. This has only started today. I re-loaded the game 4 times, but still the same. I also did a repair. No difference. ????? No CC or mods.

It's still the same unfortunately. I created a new save, that was fine. It's just my Innisgreen save. Don't know what's going on.

Try starting a new save game, play a household for a little while, then save the game and exit the Sims 4 game. Then load up the game again, and see if the household from the new save game on the main screen still is bald. If they aren’t, then load your other save game, play for a while, save the game. Then when loading the main screen again, see if the issue remains.
