What is this High School Years CAS item?

 Glad you found a solution. I do have a small bit of info that might be helpful to players if they come across this.

I have encountered this bug on other people's uploads to the gallery before, and from what I have seen, for some reason the game tries to apply acne to sims sometimes but it does not show up (perhaps some sort of invisible modifier?) This makes it look like it's using HSY in the gallery, but can be downloaded just fine without it. I don't know what causes this exactly. It would be something to investigate, I think. I'm not sure how to remove it other than disabling the pack since the invisible acne is not selectable through CAS as far as I can tell.

Yes, I believe I removed the gender filter everywhere I could, still no luck :(

xr0xt4r​ Did you check the female items?  It might be something that is either for both genders or is a female item that got inadvertently placed on your male sim and is more or less invisible if it is something small.

Unfortuntely none of them seems to be the one :/ I checked every hair, facial hair, makeup, skin detail (face and body), clothes and accessories (both face and body) for each outfit category. It's a male young-adult Sim.

Possibly an a skin detail ? Maybe it's acne ? I can't really tell from your pic and cant' find it in CAS either. What age group is your Sim ?

It is a CAS item that came with High School Years pack. This is a screenshot from the EA website which shows some of the CAS items that the pack includes
