So in the real would it is often fairly easy to confuse which twin is which when they're identical (and if they're trying to be mischievous), which made me curious. When there are identical twin sims in the game, do other sims become confused as to who is who or do they recognize the names and just go, "oh yeah that's twin 1 and that's twin?"While I am sure that it is the latter (which would make me a little sad), I thought I would ask and see.

I have identical twins, who are grown up now, but as they were growing up, I could always tell them apart, even if people who did not know them well might not.

I have two twins in the game. I can’t tell them apart. But apparently the NPCs can, because they never get them mixed up.

There is no animation for "oops, I thought you were my boyfriend but sorry I kissed the wrong twin".

My sims tend to have twins in spite of my never buying the reward trait or living on a ley line. I can't stand it because I prefer to avoid playing kids so when I let my sim have one, I only want ONE! :p I have to do their hair and clothes completely different. Like, one will have long hair and one very short. And I even color code all their wardrobe so I can identify them by which one is wearing blue and which one is wearing yellow or some such. It is so obnoxious.

ETA: However, for story purposes, I could totally get behind a love interest not being able to tell and it causing drama. LOL

I'd love such a feature! Especially since I have not just a pair of identical twins (Erwin Pries' and Julia Wright's kids) at the moment, but also a number of androids who are literally identical - ample opportunity for cases of mistaken identities!

Maybe the more traits a sim knows about a twin in question, the less likely they'd be to confuse them? Like, Daniel and Simon might look identical and also have the exact same voice, they are even both family-oriented, but one is a hot-head and the other paranoid, what should account for different ways of talking.
