I have this exact same issue.

What expansions, game packs, and stuff packs do you have installed? Get to Work Romantic Garden Stuff, Seasons, Get Famous, City Living, Cats and Dogs, Get Together, Parenthood, Spa Day, Laundry Day, Kid Stuff
Steps: How can we find the bug ourselves? When you open the create a sim screen.
What happens when the bug occurs? When I enter create a sim, it's just a default screen and I can't see the sim.
What do you expect to see? The sim and customization options.
Have you installed any customization with the game, e.g. Custom Content or Mods? No.
Did this issue appear after a specific patch or change you made to your system? Yes
Please describe the patch or change you made. I haven't played in a while but I just downloaded the most recent patch and experienced this problem.
I also have this issue - just updated my game today. The only difference for me is that I do have some custom content downloaded from the Community Gallery (lots). However I haven't had issues until today - after updating to the most recent version.
I have the following expansions/packs: 4 Seasons; City Living; Get Together; Cats & Dogs; Get to Work; Realm of Magic; Parenthood; Spa Day;Outdoor Retreat; Toddler stuff; Bowling Night stuff; Kids Room Stuff; Vintage Glamour; Luxury Party stuff; and Cool Kitchen stuff.
