Sims can't access easy accessible Items

On 3 seperate occasions in the last 24 hours my sims has indicated they can't access easily accessible items or people. First I was playing the grim reaper career and when investigating I needed to listen to plea, the sim that had this interact my sim was stood directly in front off but indicated he couldn't reach her, I reset all sims but still didn't work, I couldn't then leave work until I wrapped things up. Second my sims baby was starving in the crib, a crib they have accessed many times but he wouldn't feed her indicating it was inaccessible. Third was a sims birthday party I wanted my sim to cook cake and he started the action but then stopped half way through couldn't resume because something was blocking him, I tried resetting items and sims, sold and purchased new counter tops oven and fridge but it still happened. I contacted tech support and the advised to clear the cash which I did, I also closed the game, restarted my PC and he still can't make cake. This game is becoming unplayable for me because of this, I can't hit targets, goals, tasks if my sim can access items to do it. Also I use no Mods so its not that.
