I have seen this happen on a number of my Sims. Changing the Sim's appearance in CAS does not make the change happen on the Special outfit (used for school, work, etc.). The only way I have found to fix this is to enter cas.fulleditmode and make the changes in the Everyday outfit, which will then change the Special outfit appearance as well.

Could you please provide the following information
- Are you using mods/cc in your game ? Please remove them and test without them if you do. Please pick a thread tag for that.
- Does this happen to all Sims or just specific ones ?
- Did you get any through Cupid's Corner.
- If you take them to CAS, did they change there too ?
- Do they change back in their regular outfit ?
- How did you change the facial ? With active cheat or did you change them when changing the Sim from a gallery Sim ?
- Did you use the copy function at any point ?

- no mods/cc to remove or test. i'm fairly new to the game all things considerd
- it happens to one specific sim - a child born to my initial sim. when she aged up, i adjusted her hair style and gave her glasses. no facial feature changes whatsoever. i changed her outfit through CAS and things.
- when she returns home, she remains in her "school" clothes, which includes the incorrect hairstyle and removes accessories.
- i can change her outfit to something and it reverts correctly.
