The Dive Club came with Island Living. It only generates if you have less than the minimum number of clubs. It has always had a broken icon. The only way to "fix" it is to have your Sim take over the leadership and choose a different icon. The diving activity doesn't really do much, so as a club it's a bit of a bust. However it's can be annoying to see those broken purple icon every time your Sims go to a beach.
