I also encountered the same issue.
Windows player.
I own all the packs up to this date (23.10.2025) Game version 1.118.257.1020
I use mods for my main save.
I did 2 test: 1st my main save but removed mods and 2d created a new save while mods were out and cache was clean.
Based on the above, the issue is present in both tests. See details bellow.
What I mean by *Default outfit - just the first premade outfit in the selection for the role.
Test 1. Main save - mods removed, clear cache.
Host (elderly lady) is wearing a random outfit from the selection of outfits. The options that are set up clearly shows different.
I went ahead and switched to a different outfit—it saved for a moment, but as soon as I left Build Mode, and then returned back to a Build Mode - it reverted.
Host also didn't change the outfit when I exit to a Live Mode.
Test 2 - New save file, no mods, clear cache.
Similar behavior, observed 2 waiters upon loading on the lot : one is wearing the correct outfit, the other wearing the random one from selection.
I changed a male outfit to another option. The game immediately switches his outfit when I load into Live Mode; however, when I return to Build Mode, it reverts back to the default outfit.

there's been a few different issues with uniforms over the years so just making sure we get the correct one.
If you design your own, does that stick or also not ?
