My newborns aged up to infants, and along with their first achieved milestones came double notifications.
The milestones I’ve gotten so far, divided between my two infants: "smile", "first guest" and "babble"
I’m not sure if it’s all milestones or just these, but I assume it’s all.
New save, new sims. Mods removed.

Update:
More milestones with double notifications: "Stretching", "first blowout", "peed on caregiver", "lifting head"
I’ve given both infants bubble baths. One got two notifications for "first bath" and one notification for "first bubble bath", while my other infant got two notifications for "first bubble bath" and one notification for "first bath".
Got a single notification for "First laughter".

Oddly, I don't think it's all infants, or at least not all milestones, nor all the time. I had a household with 7 infants and at first milestones weren't duplicating, but the last time I played more duplicated than not. There were still a few that came in as only a single notification. I'm unsure if it was the infants themselves, or just the higher level milestones - I think the handful I saw that were still single notifications were from a couple of the infants that were a little further behind the others.

After updating my game and installing the Royal and Legacy packs, I receive a double message about achieving an infant milestone.
