I, too, think this is across the board. I've noticed it with the Beekeeper Hat, the Halloween costumes and the Ranch Hands aren't wearing hats.
I play on PC, am fully patched and have never used mods or cc. I didn't notice the issue until the July 16, 2025 patch.
