I'm so glad I found this thread! I googled the issue to see if it was happening to anyone else. I thought it was just my game because I do use CC/Mods but it looks like it's not mod related if it's happening to Console players and PC players with no mods. I've been using mods for years and I've seen a lot of things, but I've never seen them just not wear the hats to their costumes. When my Sim's put on costumes for Halloween it was a huge problem because none of the Star Wars Costumes make any sense without the helmets.
So far, I've seen it happen with:
- Halloween Costumes
- Lovestruck Costumes
- Geek Con
- Lobster Larry (My Sims go to Foxbury, but I imagine that it would happen to Darby as well)
- The Diver Career Uniform
