The missing headgear issue is happening in my game too, in multiple saves which are either completely vanilla or saves in which I had mods and cc at one point and then removed them. I play on PC and own all the packs.
The NPCs I've noticed missing their headgear so far are the Firemen/women (who visit when a fire is on my lot), Police Officers (who visit during a Burglar visit) and all the cos players at Geek Con. I've attached a pic from Geek Con. I hope this is fixed soon, it's totally ruining immersion!
