Since before the recent patches, my ranch hand spends most of his time using skill items (instruments, easels) or drinking water than tending to my livestock. So much so that my animals regularly threaten to leave and only stay with the intervention of my sims, pretty much defeating the point of a ranch hand.
I play an unmodded, cc free game on Mac. I have all the packs except SW:JTB, and some kits.

Community ManagerMy Ranch Hands keep dying because they'd rather swim in winter than do any work with the horses.
My ranch hand has been playing with my baby's toys or, after specifically directing them to check the animals or do gardening, running to the far corner of my farm and then coming back to play with the baby toys.
I specifically asked my ranch hand to collect horse poop and she didn't. She does brush the horses and spend time with them though.
