The memorial portraits from the Life and Death Expansion Pack appear to be bugged as they no longer generate an image of a deceased sim, instead showing a blank white image.

This JUST happened to me and I'm devastated because it was my third generation heir that just passed away and he was one of my favorites. I've had his gravesite all set up since I moved out with the fourth generation heir (who is almost an elder herself he lived a very long time) and he just passed. The fix of "oh just put a picture in there" doesn't work when the pictures all glitch out and go black so I haven't been taking any. Now I have no photo for this sim and I'm sure that when it's fianlly "fixed" it will be too late to have him attached to it because eventually your ghosts disappear and can't be attached anymore.
If you have them as ghosts, just keep them in your household or another household until they repair it. Unfortunately there is no fix for it. I had two sims that passed away, I made funerals for them so that their families can move on and when they fix the game I will put the photos and send them to the other side. This is the only solution that I can think of. If you keep them in your household I think you can add them later, as they will be considered playable sims.
