The memorial portraits from the Life and Death Expansion Pack appear to be bugged as they no longer generate an image of a deceased sim, instead showing a blank white image.

I am also having this issue. Repairing the game did not work for me. Dragging family photos into memorial portrait is also not an effective workaround off the home lot because the memorial portrait item resets every time I return to the cemetery.
