The memorial portraits from the Life and Death Expansion Pack appear to be bugged as they no longer generate an image of a deceased sim, instead showing a blank white image.

I have found that changing to DirectX9 was able to immediately fix my issue, albeit I am using mods/CC. I set to DirectX9, restarted my game, and then set the sim again for the portrait, and it worked immediately.
