I have done this in my game with mods enabled and disabled.
Whether it is under an arch or not, my Sims will exchange vows, but the UI in the wedding ceremony remains unchecked. They even have the moodlet from exchanging vows, but it still does not work.
I'm not sure if anyone else is experiencing this problem.

Had my sims customize a wedding ceremony with Walk Down Aisle, Exchange Vows, Spousal Kiss, Throw Rice, Blow Bubbles, Cut Cake, First Dance and Dancing in a modded game. Everything except Exchange Vows worked. Tried to exchange vows at the wedding arch twice more, thinking it had glitched, still Exchange Vows wasn't checked off though they went through all the motions. Threw together an engaged pair in a new, unmodded game, and again Exchange Vows did not get checked off during a customized wedding ceremony.

I did move my mods to another folder on my desktop when I originally posted. I never deleted the localthumbcache.
I have MWS stories.
I just tried again after deleting the local thumb cache with no mods in my mods folder and it still is bugged. :(
After the Sims exchange vows and kiss, they glitch and go into a T-Pose. They do get the exchanged vows modlet and spousal kiss modelet.
I encountered the same problem, an attempt to get married using the arch after the wedding party helped, apparently the event itself breaks the marriage or the action from the My Wedding Story package.

is this a wedding from MWS story or base game wedding ?
There is a known issue where the option is greyed out but that sounds different.
Did you also try moving your mods folder to the desktop and deleteing localthumbcache file instead of just disabling ?
