Having the same issue. It's multiple sims. It's when I click on the toilet - the option just doesn't show up 99% of the time. I've gotten it to show up once as an option, and my sim was pregnant, but when I went to manage worlds and came back (did not even switch active households), the sim was no longer pregnant.
