I'm having a similar issue with pregnancy. Got an NPC Sim pregnant with try for baby, saw her showing pregnancy thought bubbles, saw her throw up but her body never changed, could still do try for baby command, then suddenly she called and said she had given birth to a baby. I don't have any mods, I'm on pc
