No Pregnancy test to take & other pregnancy issues

You can workaround this bug, by interacting with the toilet or other bathroom items a couple of times. I tested it with a material sim, and used "clean toilet" and "view new object" interactions, and for the third click, take pregnancy test popped up. A friend of mine, got it to pop up, after her sim washed its hands after try for baby. The bumb will also look normal if done this way.

You can also see if a sim is pregnant, by moving your mouse over the little icon for the next age state, in the simology panel. It will say something like - pregnant sims can't age up.
